Wild Poliovirus Outbreak in Kenya Situation Report – 8 August 2013

 

Highlights

  • Kenya’s number of confirmed cases of WPV type-1 in 2013 remains at 10. Date of onset of the most recent case is 6 July 2013. All cases are from the Dadaab area: 7 cases in the refugee camps and 3 outside the camps in the host community.
  • 1 death reported in a confirmed polio case, an adult aged 23 years. There are two other adult cases confirmed, both aged 19 years.
  • Round 4 of Kenya’s immunization response to the current outbreak will be conducted 17-21 August 2013 in 24 counties targeting 4.9 million with bOPV.
  • Kenya’s National Polio Steering Committee met on 31 July, chaired by MOH Director of Public Health, Dr S.K. Sharif, to review current response strategies. The country’s SIA calendar through September was endorsed.
  • The 3-Month Polio Outbreak Assessment (external review) is proposed to be conducted from 15-26 August in Nairobi. Reviewers are expected to observe/monitor SIAs as part of the review.
  • Somalia has reported 5 new cases in the past week bringing to 100 the total number of cases of confirmed WPV type-1 to date from 9 regions. Date of onset of the most recent case is 10 July, from Balad District, Middle Shabelle.
  • A cross border meeting of local MOH and NGO health teams from Ethiopia, Somalia, and Kenya was conducted in Mandera, Kenya on 2 August. The meeting report and action plan for follow up will be distributed 8 August.
  • A cross border meeting of teams from South Sudan and Kenya was conducted in Kapoeta, South Sudan on 7 August to coordinate vaccination activities of all cross border populations.
  • Two cross border meetings between Uganda and Kenya (one each for northern and southern districts) have been postponed until the week of 9 September, final dates to be confirmed.

 

Epidemiology

  • Kenya’s polio cases reported so far are in the Dadaab Refugee Camp and the camp’s host community. The ongoing explosion of cases and geographic spread of poliovirus in Somalia continues to present high risk of further spread into Kenya. 

 

Vaccination response

  • Round 1 conducted 27-31 May 2013

-          Target pop 512,610 <5 years in all districts except refugee camps and 3 host districts where <15 years were targeted.

-          Antigen: bOPV in Dadaab refugee camp and 3 host districts, tOPV in the rest of Garissa

-          Coverage: 95% administrative and 86% IM.

  • Round 2 conducted 17-21 June 2013

-          Target pop 1.35 million

-          Coverage: 101% administrative and 91% IM

  • Entire pop in Dadaab Refugee Camps
  • Under 15 years in 3 host districts
  • Under 5 years in 22 other districts
  • Round 3 in three phases during 1-10 July 2013

-          4,661,881 million were vaccinated

-          IM data from finger marking indicate national house to house coverage of <5 years at 93.3%; parents awareness of the campaign at 85.7%; and parents awareness of the disease being vaccinated against at 78.3%.

-          Antigen: bOPV

-          All ages: Completed in Dadaab Refugee Camp (July 1-6)

-          <15 years: Completed in Four districts borderingDadaab Camp – Dadaab, Fafi, Lagdera, Wajir South(3-7 July)

-          <5 years: All other districts

-          Scope: 24 counties (127 districts)

  • Phase One: 1-5 July (Dadaab camp)
  • Phase Two: 3-7 July (Predominantly Muslim communities)
  • Phase Three: 6-10 July (Predominantly non-Muslim communities)
  • Mop-up vaccination with bOPV was conducted 27-31 July targeting 25,787 children <5 years around the latest confirmed case of polio in North Eastern province. The divisions targeted are Dadaab, Liboi, Jarajilla and Sabuli.
  • Round 4is planned for 17-21 August 2013

-          Target pop 4.9 million

-          Antigen: bOPV

-          Scope: 24 counties (127 districts)

-          Discussions underway for coordination and synchronization with neighboring countries.

  • Round 5 (21-25Sept SNID) proposed in the same area& target population
  • Round 6 (tbc 2-6 Nov NID, tOPV)
  • Round 7 (tbc 7-11Dec NID, bOPV) 

 

Advocacy and Coordination

  • Preparations for two national launches of Round 4 in Nairobi and Garissa are on-going.
  • Printing of 3,000 copies of the Advocacy Kit is expected to be completed by 10 August.

 

Communication and Social Mobilization

  • Funds for county and district advocacy and social mobilization have been in the counties since the last round to allow early start of social mobilization activities.
  • At national level, messages for radio and TV have been reviewed and updated. Production of the PSAs will be finalized this week ready for broadcasting from 12 August.
  • Printing of IEC materials is expected to be completed by 12 August for distribution by national supervisors.
  • A meeting is scheduled with the Nairobi team on 14 August to discuss progress with implementation of strategies for the hard to reach populations.
  • A comprehensive ACSM strategy with budget for resource mobilization is under development for inclusion in budget presentations at the HOA polio donors meeting in September.
  • A training module for national supervisors on communication, including monitoring and reporting, has been developed for supervisors training in Nairobi on 12 August.   

 

Security

  • An early morning fire on 7 August, in the international arrivals terminal of Jomo Kenyatta International Airport in Nairobi, suspended all but emergency inbound and outbound flights. The cause of the fire is unknown and no injuries have been reported. 
  • As in previous reports, insecurity due to inter-clan clashes remains a recurring issue and will be monitored regularly. 

 

Resource mobilization

  • A HOA donor meeting is to take place on the 6 September tbc in Nairobi. Anjali from UNICEF/HQ will support preparation and concept note development. 
  • The UN Foundation/UNICEF delegation visiting Kenya was briefed on the HOA outbreak and response by GOK, WHO, UNICEF, and partners. The delegation lead, American actress Amanda Peet, expressed willingness to advocate for additional resources for the outbreak response.
