As we move into the final chapter of polio eradication, we may wish to think back on the years since 1988. Could the billions spent on OPV mass campaigns have led to a more rapid conclusion if some of them had been spent on building up routine immunization? As GPEI comes to a conclusion, will measles/rubella eradication learn the right lessons about resource allocation between RI and SIAs? Will those donors – that donor – which spends billions on new and underused vaccines put more money into immunization systems strengthening, a window formerly open? If not, what will be the benefit of one or two doses of the new and underused vaccines, purchased at great price and either frozen through cold chain underinvestment or through poor completion rates?
